Your Centre has expressed interest in conducting modified training & competition, and as a pre-requisite of doing so, it must register a minimum of one COVID Safe Official (CSO). 

This registration process must be completed in full by the nominated Centre COVID Safe Official.

Details required to be provided during this application include:

• Name (first & last)
• Email (personal not centre)
• Mobile Contact
• Centre 
• Club Name (required only for Centres who are nominating a representative for a Club at Centre level)
• Committee Role (if applicable)
• Childsafe Credential you hold (WWC. VIT or Police) its registered number and expiry date (WWC, VIT) * please note those who hold a WWC or VIT should have LAVIC - (your Centre) / your Centre listed as a provider with WWC Victoria.

Your Centre will be notified in writing upon successful completion.
