This form is to be completed in order to be covered by insurance if assisting as a volunteer within your local Little Athletics Centres sanctioned events.

This document should be used for all volunteers assisting at the Centre in the case they are not a parent or guardian listed in the registration process of an athlete (parent 1 or parent 2), ordinary members need only complete this document once

All Ordinary members over the age of 18 must hold one of the following:

• a valid WWC card

• a valid Victorian Institute of Teaching card (VIT)

• A registered Victoria Police officer or an Australian Federal Police (AFP) officer badge

Cover starts from the registration date.
