This form is to be used for people renewing after a lapse of at least 4 months in their previous membership.  You would first make your payment via the online methods or direct to the club and then fill this form out within 7 days of that payment.

If you choose this method of renewal you will have a break in membership recorded against your record and will receive a full 12 months of membership from the date of renewal.
